The Sales Disclosure form application is available on-line at the DLGF site.
The SDF must be stamped by the County Assessor before bringing it to the Auditor's Office.
There is a $10.00 fee when filed in the Auditor's Office. No fee for an exempt transaction.

Affidavit
Effective November 1, 2011, the Auditor's office will no longer "PREPARE" auditor affidavits, nor have the form. The person whom title has passed to is responsible for the preparation and filing of the affidavit with the Auditor's office.
It isn't necessary to bring the affidavit into the office it can be mailed. See the address below. However, it must by notarized.
