Payroll Address Change Form

The form provided on this page will only change your address with payroll. When you send this form to the Bookkeeping Department of the Auditor's Office, you still need to change your address with:
  1. your health, dental, and vision insurance
  2. and your retirement plan.

Also, remember to contact Jane Laib for any changes on any dependents on your insurance policies.

If you are a Sheriff's Deputy, under the Sheriff's Retirement Plan, you need to contact Steve Woodall, Sheriff's Pension Secretary or contact John Strange.


Form for Change of Address
(Fill out on-line and print)

Steps to submit this Change of Address request:
  • Fill out your data on-line. Advance to the next field by using the Tab key or the mouse.
  • If you make a mistake and want to clear the form, use the Reset button located at the bottom of the form.
  • Once your data is entered, use the Print & Reset button to print your form. Please follow instructions at the bottom of the form. Be sure to use the Print & Reset button at the bottom of the form or your data will appear to the next user of the form.
  • Sign your form and send in the inter-office mail or fax (435-5027) to Jane Laib in the Bookkeeping Department of the Auditor's Office.
